This questionnaire was created to help us serve you better. Please answer each question with as much detail as you feel necessary in order to help us do our best for you. Our goal is to provide you with the very best fishing vacation you could ever have in Southwest Alaska. Please be assured that all replies are strictly confidential.
1. Our cabins are double occupancy. List any special cabin requests, special needs or roommate assignments
2. List any special dietary requirement, i.e. diabetic, vegetarian, food allergies, low/nonfat or low/non salt diets
3. List any medical and/or physical conditions we should be aware of (This is very important. Please include any special medications you are taking such as insulin, heart medication, blood pressure, medical allergies, etc.)
4. For our flight planning purposes as well as the R/T charter manifest for Lake Clark Air, please provide us with your height and weight
5. Please indicate what style of fishing you prefer: ______ Fly-fishing or ________ Spin Fishing (As an aside, we strongly recommend chest high breathable waders and wading boots with felt soles for the best in safety, comfort and versatility. Give us a call if you need help locating waders)

NOTE: As an aside, we strongly recommend chest high breathable waders and wading shoes with felt soles for the best in safety, comfort and versatility. They are available at most local and/or large sporting goods stores. They are also available through Cabela's or Bass Pro catalogues.
